Please complete Start of Year Forms as soon as possible and return to school no later than September 30. You must print or save and email them to arpoffenbarg@seattleschools.org.

These are the forms that are required to be returned: 

1. 504-2 to Identify Disabled Students
2. FERPA (PreK-8 or HS versions)
3. Housing Questionnaire (Only return if applicable.)
4. Title VII Indian Education (For new students only)

Please also update your information on your Source account by September 30th.
